Cause - Consequence
The consequence here is all the bad we make; as a whole and as individuals. The questions are just two. Where does it come from? My answer is: "I". Selfishness, or the power of the I before the we. To look for oneself's good before the other's. To look for benefits for us before benefits for the people who are in front of us, to hurt deeply the ones we have next to us to feel better, to kick them off in order to be over them. Can we fix it? I am afraid I have not the answer for it. But I hope it was a yes.

I'm not sure if that is the only cause. It's a lot, yes, but 'we' causes MANY problems too, from people who don't believe individuals have rights to 'we are better than them'.

It's just the same as stated before, but instead of being one it's a group of people who care about their own asses. Selfishness, egoism,... just applied to a collective.

Just to make this more philosophical, I may add that consequence has subjective interpretations when unfolding. This may be due to selfishness or some other action.

Most have very little appreciation of all the things that are done on our behalf. We buy gasoline, get water from a tap, get food at the market and but "stuff" at the store. It takes a lot to get it there. With that comes a lot of mostly hidden damage.
